Psychiatric assessment (new patients)
The new patient psychiatric assessment is a 90 minute appointment where we explore your symptoms and challenges to begin the process of formulating an individualized, integrative treatment plan. I want to hear from and understand YOU. Whether you have been diagnosed with a psychiatric condition and have tried a dozen medications, have never seen a psychiatric provider, or fall somewhere in the middle, we will take our time to connect and I will devise a treatment plan that works for you.
During the evaluation, we look at numerous factors that could be contributing to your current symptoms including, but not limited to:
Past trauma(s)
Psychosocial stressors
Nutrition/nutrient deficiencies
Genetics
Medical conditions/overall health and wellness
Sleep
Movement/activity level
Relational challenges
With the new patient appointment, my goal is that we have established the foundation of a therapeutic relationship, I have had the opportunity to perform a comprehensive assessment, and we have clarity in moving forward together. I will provide actionable steps after this initial meeting for you to implement; however, in some cases, I may need to gather more information such as labs prior to prescribing medications and other interventions. In all cases, I want you to leave feeling hopeful and empowered.
Follow up appointments (current patients)
Follow up appointments are 30-60 minutes in length, giving us time to discuss your progress between appointments. We discuss the changes you have noticed since our last meeting, and we implement new strategies as needed to support your healing process.
Tools that may be discussed during and utilized after these appointments include, but are not limited to:
Medications
Targeted labwork
Nutrition, medical grade supplements, and herbs
Supportive psychotherapy
EFT/tapping and energy medicine
Mindfulness, meditation, and breathwork
Sleep hygiene
Movement/physical activity
Follow up appointment frequency varies depending on your needs, but all patients must be seen at least every 3 months to be considered a current patient and to receive prescription refills. Those on controlled substances such as stimulants and those withdrawing from benzodiazepines will need to be seen more frequently.
Paperwork and miscellaneous services
I am happy to provide letters and documentation as needed and appropriate for current patients for emotional support animals (ESA), school/work absences, etc. For documentation requests, a $50+ fee may apply, depending on length and complexity.
See more about ESA evaluation and documentation here.
Insurance/Billing
Currently, we are contracted with Optum/United Healthcare, Aetna, Cigna, and BCBS. Please inquire to verify that we are contracted with your specific plan. For any folks with insurance plans we are not contracted with, we are happy to provide a superbill for you that you may be able to submit to your insurance company for reimbursement. Please check with your insurance company to determine if you have out of network coverage for psychiatric care. Cash pay rates for services are $275 for evaluation and $150 for follow up appointments.
For Aetna and Cigna: use Alma to verify insurance
For Optum/United Healthcare and BCBS of AZ: please contact us directly to verify insurance
When emailing about insurance and scheduling, please include a copy of your insurance card front and back along with your date of birth and phone number so that we can verify your insurance coverage and benefits and add you to our EHR system.
Policies
We work with you to order labwork at labs contracted with your insurance plan if applicable, such as Sonora Quest or Labcorp. We also utilize Rupa Health, a company that facilitates discounted cash pay prices for standard and functional labs. Additionally, we offer access to discounted nutraceuticals via Fullscript, a professional grade supplement dispensary.
With a signed Release of Information (ROI), we are happy to collaborate with your therapist, primary care provider, and other members of your healthcare team.
Each practice and provider has a different approach to prescribing controlled substances such as stimulants and benzodiazepines. Generally, we only prescribe benzodiazepines for patients in the process of de-prescribing and withdrawing from these medications or for very limited use such as infrequent flying. If you are looking to continue on benzodiazepine medication long-term, we are probably not the right fit for you, but please contact us to discuss. Stimulant medications may be prescribed if appropriate, though we will also discuss non-stimulant options for treatment.
We maintain a cancellation policy that requires 24 business hours notice should you need to cancel or reschedule your appointment. If notice of at least 24 business hours is not made, the credit card on file will be charged a $150 late cancel/no show fee for evaluations and a $100 late cancel/no show fee for follow up appointments. We appreciate your understanding and respect of this policy so that we may best serve all patients.
We do not provide worker’s compensation evaluations, forensic evaluations, or provide legal services/testimony.
